Eligibility Rules

Determine Your Benefits Eligibility

Employees

Full-time employees are eligible for the Company’s benefit plans after 30 days of full-time employment.

The 401(k) retirement plan is open to all employees age 21 and over, after 1,000 hours employment.

Eligible Dependents

Eligible dependents of full-time employees include your legal spouse and children up to age 26. The definition of “child” includes any of the following:

  • Your child / stepchild
  • A child placed with you for adoption
  • Your legally adopted child
  • A child for whom you have legal guardianship
  • Your child for whom health coverage is required through a Qualified Medical Child Support Order (QMCSO)

Changing Your Status With the Company?

The scenarios below explain changes and benefit-related tasks that need to be completed.

You will have an action item in your Workday inbox to complete your benefit elections. This action item MUST be completed within 14 days, even if you have insurance elsewhere.

Benefit coverage will begin after a 30-day waiting period, in most cases.

To enroll any dependents, you will need:

  • Marriage certificate for spouse coverage
  • Birth certificate(s) for child(ren) coverage
  • Social Security cards for all dependents

NOTE: Enrollment in Basic Life, Short‐Term Disability and Long‐Term Disability is STRONGLY encouraged. Learn more about these benefits and/or talk with your manager about the importance of electing these benefits.

  • There is no waiting period for benefit coverage to begin for an employee rehired to a full-time position if the following conditions are met:
    • The most recent employment with the Company was in a full-time position for at least twelve consecutive months.
    • The rehire date is less than twelve months since the termination date.
  • If you meet the conditions above, the Benefits team will contact you shortly after you are rehired to explain your eligibility for no waiting period to restart benefits. Or, feel free to contact them for more information.

Otherwise, benefits for a rehire are treated like a regular full-time hire; see details in the section titled Promotion to Full-Time / Full-Time Hire above.

Health Insurance Coverage

Employees who have health coverage will be covered through the end of the last day of their full-time employment. For example, if your last day as a full-time employee is January 15, coverage would end at midnight on January 15. You will receive a letter regarding setting up optional COBRA coverage shortly after your employment terminates.

HSA and 401(k) Accounts

The funds in these accounts are yours, but you may be responsible for account maintenance fees. You are free to transfer or rollover these funds within the limitations of the account type and IRS regulations. Other rules or limitations may apply.

Other Benefits

Most other benefits will also terminate on your last day of full time employment. You will be notified by mail with instructions for any benefits that have an option for continuation at your own expense.
