Form 1095-C is a new, year-end statement for employees covered under Discount Tire’s medical plans, scheduled to be sent in the mail this coming March.
- Your 1095-C contains detailed information about the health care coverage provided in 2015 by Discount Tire’s medical plans.
- You should keep Form 1095-C with your other important tax records, though it is not needed to file your Federal and State tax returns for 2015.
Note: You will need to file Form 1095-C with your tax returns beginning tax year 2016.
